Acute Zonal Occult Outer Retinopathy without Photopsia

Abstract

|
A 30-year-old woman developed headache and acute visual field defect in the left eye. Examination revealed that visual acuity was 0.63 in the right eye and 0.16 in the left eye, with a left relative afferent pupillary defect. Anterior segment examination and fundoscopy were normal. Humphrey perimetry showed loss of temporal fields with relatively sparing of central fields in the left eye. Fluorescein angiography, visual evoked potentials, and orbit MRI were unremarkable. Multifocal electroretinography (ERG) demonstrated functional disturbance of the nasal retina in the left eye, and optical coherence tomography (OCT) disclosed the distruction of the boundary between the inner and outer segments of the photoreceptors. Intravenous steroid treatment did not improve her visual field defect. Acute zonal occult outer retinopathy (AZOOR) should be considered in patients with atypical optic neuritis, even though there was no associated photopsia.
